Serious Process Incident (SPI)
Serious Process Incident (SPI) : A process incident that results in a fatality, multiple lost work days, or any of the following actual off-site impacts are classified as automatic serious process incidents: evacuation, non-precautionary shelter-in-place, injury, major media coverage, significant damage to property, significant community impact, or overnight hospitalisations. There are specific definitions in the KPC HSE system namely, an incident that scores 130 points or higher using the scoring system
