Accident 45 : General. An unwanted transfer of energy or an environmental condition which, due to the absence or failure of barriers and/or controls, produces injury to persons, property, or process (SSDC); as defined in NHB 5300. 41(1D-2), "An unplanned event which results in an unsafe situation or operational mode" (NSTS 22254); an unplanned and sometimes injurious or damaging event which interrupts the normal progress of an activity and is invariably preceded by an unsafe act or unsafe condition or some combination thereof. An accident may be seen as resulting from a failure to identify a hazard or from some inadequacy in an existing system of hazard controls (ASSE)
