Barrier Failure :
A barrier is considered to have failed when it no longer provides the protection to facility and site personnel, the general public, and the environment afforded by design or operational controls. Failure of a barrier can usually be recognized by the readings or output from plant instruments such as valve position indicators, failed fuel monitors, pressure sensors, or stack effluent monitors.
