Consequences 4 : In aviation, usually described qualitatively with words like Catastrophic, Hazardous, Major or Minor. Examples of a Catastrophic include loss of the aircraft which leads to a multiple fatalities. Examples of hazardous include a large reduction in safety margins, consequenly leading to physical distress or a workload such that the flight crew cannot be relied upon to perform their tasks accurately or completely, with the consequences leading to a serious injury or death of a relatively small proportion of the occupants. Examples of major include a significant reduction in safety margins, consequenly leading to a reduction in the ability of the flight crew to cope with adverse conditions as a result of increase in workload or as a result of conditions impairing their efficiency, with the consequences leading to a injury of occupants. Examples of minor include nuisance, consequenly leading operating limitations, with the consequences leading to emergency procedures
