Incident 03 :
(1) An unwanted occurrence which involves no injury, damage or loss but which, under slightly different circumstances, could have these consequences - a near miss. (2) A special definition on incident is used in MORT analysis. It is the failure of a control (barrier) without consequences. The acronym MORT is used to refer to four things: (1) a safety-assurance programme which ran between 1968 and 2002; (2) the body of written material which documented the programme; (3) a logic tree diagram: the Management Oversight and Risk TREE; (4) a method for helping investigators probe into the systemic causes of accidents and incidents
