Informed Consent 3 : This will be required for procedures the health professional may undertake as part of the baseline health assessment, or as part of the follow up, for example, immunisation and pathology tests. Informed consent requires the decision-maker (the child if 'Gillick competent' or the child's guardian) to have information about the procedure and have the opportunity to discuss concerns with a health professional before providing consent/signing the consent form
