(ICT) A data entry form is often used to input data, which has previously been collected, into a database. There are some common controls within a data entry form that include: (1) Text Box: Allows a single text line to be entered. (2) Text Area: Allows multiple lines of text to be entered. (c) List Box: Allows you to pick one or more items from a list. (d) Combo Box: Allow you to pick a single item from a list. (e) Radio button: Always in a group of at least two, but often more. radio buttons allow you to make a single selection from a number of options. (f) Check box: A small rectangular box that allows you to select an option. (g) Submit Button: Allows you to send the data. Often some 'validation' checks are made before sending in order to make sure the data format is valid and within range
