Evaluation Report : An evaluation report describes the intervention and its aims and objectives; the methods used to evaluate it; and the results found. It often includes a discussion of the results along with recommendations for further work. An evaluation report has a standard structure of: Introduction, Methods, Results, Discussion, Recommendations, Conclusion, References, and Appendix. The exact structure of the report though will depend upon whom it is being written for
